Default Bug (just FYI)

I had once in omaha tutney one bug wich took 2000 chips from my hand.
I was leading with chips within last 3 players.. I won another hand but when next hand started I had exactly 2000 chips less. All other 2 players in the table wondered also that where did my chips dissapeared.
I finished third (probably because of that) cos after chips dissapearing I had less chips in my hands than other 2 players.
In the end of the game there was only 1500 chips per whole table. Do the calculations.. it is not possible in tourney that by the end of the game there is so few chips on the table (and no one did not steped out of the game).
Later on I've been told that I should write down the table and hand no. when it happened and ask for some compensation in the forum but I didn't know that at the moment so I have nothing to prove it with anymore.
Question to moderators / admins:
Is it really possible that such thing happens?
Have it happened before?
And what information exactly do I need to write down to get compensation?

since you placed they aren't gonna give you your buy-in back...and you have to write down the tournament name and number [top left hand side], it happens there are a few glitches but it get sorted eventually, and it has happened b4 its happened to me..and many others your not the first nor the last. other than thats good luck and see ya again on the tables

p.s. i'm sure i forgot to mention something so feel free add it
